The Canadian Foundation for Healthcare Improvement (CFHI) is launching a 17-month collaborative to improve patient and caregiver experiences during transitions from hospital to home.
Transitions are often difficult and, in health care, they can be dangerous. The Canadian Foundation for Healthcare Improvement (CFHI) is launching a 17-month collaborative to improve the quality of care, and patient and caregiver experiences of care, during transitions from hospital to home.
The project will support quality improvement teams that include patients, caregivers, providers and hospital leaders to implement and evaluate a patient-oriented care-transition bundle.
The care-transition bundle has four elements:
- A patient-oriented discharge summary (PODS).
- Patient and caregiver education.
- The involvement of caregivers throughout a hospital stay.
- Post-discharge follow-up.
The goals of the collaborative are to:
- Improve the patient and caregiver experience of transitions from hospital to home/community care.
- Improve the confidence of patients and caregivers in managing their care as they transition to home.
- Improve provider experience of care.
- Reduce avoidable hospital readmissions.
- Enhance the ability of teams to effectively partner with patients and caregivers in improvement initiatives.
